No one testing method is going to tell you everything. I've heard plenty of welders say they'd rather take an x-ray test than a bend test.

I wouldn't worry about trying to get it in a certain number of passes. If you try and fill it up too fast, say, by using wire that's too big for what you're doing, there's a good chance of getting lack of fusion and voids.
If it takes an extra pass or two, so be it. Don't try to force it.

For holes that big I would either use the lay wire technique and weave it, or run small beads on one side and keep stacking them until the hole is filled.

There is some good advice here but I would suggest that if you start changing thing just change one thing at a time. Some thoughts: Next time take a picture of your root pass before you run a "hot" pass. This will tell you/us a lot. If you do it right your "hot" pass should really only be a fill pass. Running your "hot" pass too hot could mess up your root; or fix it, it really depend...

I also weld a lot of sch 10 stainless steel pipe. To get things started when we measure pipe for cuts we never take out for the gaps and it always ends up the overall length we need. If it's an AWS/ASME/API certification they should have a WPS for you to go by. This will outline all your parameters.
There are no requirements for practice, though it is encouraged.

I'll echo what MinnesotaDave said that hot and fast is usually the best way; insofar as my experiences. If you do get an unusually/uncomfortably large gap, I've had success with bridging the gap. And with larger gaps don't focus too much on one area. Weaving tends to help. And you might try pulsing ...
