Not very familiar with excel to be honest, but would it be possible to have a 'enter your country/province/city/region ( whatever) option onto this for us fellows living outside of the united states of America?

You don't have to know anything about excel. The survey does all the work and throws all of your survey information into the spreadsheet. In the spreadsheet all you have to is either scroll the window down to look at all the responses one-by-one, OR filter the spreadsheet using the step-by-step instructions in the animated GIF that I took the time to make for everyone. If you can click a mouse, you can filter a Google Spreadsheet.

Adjusted the "State" to include "other". Please input the currency abbreviation into the "Cost" field.
